Vendor Approval Process
Welcome to the Moorpark Unified Approved Vendor page. To be compliant with all rules and regulations and become an approved vendor for MUSD, please provide the following documents to Marisa Magana. If you have any questions, feel free to contact Marisa at 805-378-6300 ext. 1401.
Please note the process to become an approved vendor may take anywhere between 4-8 weeks.
FOOD VENDOR (Example: Shaved Ice vendor)
To be an approved food vendor please submit the following:
- Service Agreement
- Certificate of Insurance with endorsements - Sample
- Ventura County Environmental Health Permit
- Ventura County Seal of Inspection
OTHER SMALL VENDOR (Example: Bubble Assembly vendor)
To be an approved vendor please submit the following:
- Service Agreement
- Certificate of Insurance with endorsements - Sample
PROFESSIONAL SERVICE VENDOR (Example: Life Coach, PD Day vendor)
