Two commonly used health office forms are now on the MUSD website for your convenience:

 Authorization for Any Medication Taken During School Hours - required before any medication (prescription or over the counter) can be given at school.

Readmission to School of Student with Temporary Disability or Injury - required for any student coming to school with a cast, crutches, wheelchair, stitches, sling, etc.

Click on the links above or go to www.mrpk.org, select Student Services, then Health and Wellness to find the forms.  Printing them out for yourself will save you a trip to school to pick them up.
