Healthy Kids Survey Withdrawal Form
ATTENTION: No shots, No school: This coming year (2011-2012), all students entering into 7th through 12th grades will need proof of a whooping cough booster shot (called Tdap) before starting school. Check your student's yellow immunization card. The requirement is met if you have proof of a Tdap shot on or after their 7th birthday. Please bring in your updated immunization record to your child's school health office now.
